"Hide my e-mail addresses" should be radiobuttons, not checkbox

Bug Description

"Hide my e-mail addresses from other Launchpad users" is currently a checkbox. Unfortunately the meaning of the off state isn't obvious. (Hide my e-mail addresses from everyone?) This will become more important if bug 66105 is fixed such that admins of teams you apply to can see your e-mail addresses.

The label is also inaccurate: if the checkbox is checked and you post a bug comment, the e-mailed recipients will still see your e-mail address in the message they receive.

A better design would be something like ...

My e-mail addresses should be visible to:
(*) anyone with a Launchpad account
( ) only people subscribed to things I comment on

(It may save time to fix bug 109500 at the same time as this bug.)

We no longer leak email addresses in bug notifications. The checkbox does cause LP to hide email addresses from everyone.
