Menus do not work correctly

Bug Description
I am running Ubuntu 12.04 LTS with Unity.
When I go onto Firefox, the app's menus in the Unity panel sometimes change locations and, sometimes, the "Help" menu does not appear at at all. If I disable global menubar integration, the menus in Firefox work normally.
By change location, I mean that the order of the menus in the Unity panel changes. Sometimes, Bookmarks appears first. Sometimes History appears first, etc. The order should be (from left to right) File Edit View History Bookmarks Tools Help. So, sometimes it reads Edit History Bookmarks Tools Help View. Sometimes, it reads File Edit View History Bookmarks Tools (but not Help).
I tried reinstalling global menubar extension, Unity, Firefox. I have re-set Unity, etc., etc. This seems, at least to me, to be a bug.
