the same problem. but for usb bluetooth adapter. and problem reveals itself after reboot only. so, stepts to reproduce are:

1) boot into ubuntu and log in as user

2) insert bluetooth adapter into usb port (bluetooth will work fine at this step)

3) reboot system

expected result: working bluetooth

real result: the same as in original description.

USB Adapter is

also, looks like usb port is 1.1:

one thing more, just have rechecked..

there is no problem if adapter is connected to usb 2.0 port. So, only usb 1.1 affected for me. tested with ubuntu 12.10 and 11.10

This is reported against an old version of Ubuntu and many things has changed since then. Because of that we won't fix this issue however if this behavior repeats on a modern version please fill a bug report against it and we will take it from there.
