issue 1: A grid highlight window appear after opened a Firefox browser. (See the screenshot.)
issue 2: System froze. The application windows opened automatically after system froze. (Please see the attached video.)

The video shows the dash open. Does tapping Super or Escape close it?

Also, during the hang, could you please force a compiz core dump (and hopefully automatic crash report) using this command:
    killall -ABRT compiz

You may have to log in to a virtual terminal (Ctrl+Alt+F1) to do it. The switch back using Ctrl+Alt+F7.

@ Daniel - No, unity does not respond to Super or Escape, only the mouse pointer is movable at this time. Please note that the issue in the attached video happened when we tried to use the mouse to click on different launcher icons, for a while nothing happened and suddenly it started opening all app windows on its own and continuously flickering.
During the hang we were unable to report the bug through Apport. So far, it has not been possible to reproduce the one shown in this video. But we might still be able to reproduce the empty window (similar to grid highlight) appearing when working with Firefox and see if I can use the above suggested method to report it.
