Bug Description

The following serialization codes from W3C are never used.

There must be an error somewhere.

SEPM0004: doctype-system parameter, or standalone parameter with a value other than "omit", specified
SERE0003: serializer can't satisfy the rules for either a well-formed XML document entity or a well-formed XML external general parsed entity
SERE0005: invalid NCName character
SERE0006: invalid character for XML version
SERE0008: can not encode character as character reference here
SERE0012: the value of the normalization-form parameter is fully-normalized and any relevant construct of the result begins with a combining character
SERE0015: can not use '>' within processing instruction for HTML output method
SESU0007: unsupported encoding
SESU0011: unsupported normalization form
