I could reproduce this bug in my virtual machine by using this lines from my localstore.rdf
  <RDF:Description RDF:about="chrome://browser/content/browser.xul#main-window"
                   width="683"
                   height="716"
                   screenX="403"
                   screenY="113"
                   sizemode="maximized" />

and my ccsm settings:
desktop activated:
- Expo
- Desktop-Cube
- Viewport Switcher
- Cube rotation
- Unity plugin
--changed
---autohide
---icon size 32
effects activated
- Animations
- water effects
- groggy windows
- windows fade in/out
- window dekoration
I translated the options, they could differ from the original name.

The other settings should be the standard settings.

Could you run unity --reset and see if it fixes the issue? You have many custom plugins enabled, which could make a difference.

I checked that.
If I reset unity, the bug is gone. But if I activate groggy windows, the bug appears.
