Clicking inside a window (as opposed to on the title bar) sometimes doesn't give it focus

Bug Description
Usually, when you click on any part of a window (including its content), it is given focus and brought to front. However, sometimes (and I haven't been able to figure out the pattern), clicking inside the window area doesn't work, and you have to click on the window title bar in order to give it focus and bring it to front.
