ordering issue when flash-kenrel is used with live installer

Bug Description

during the live-installer run in the new quantal ubuntu-server images for arm, several scripts are executed that trigger update-initramfs calls. flash-kernel is executed as an initramfs-tools hook to update the bootloader setup with a new kernel or initrd. sadly at that point flash-kernel is not configured yet and live-installer seems to also miss a bind mount of /dev.

flash-kernel needs to become a no-op until the bootloader configuration stage happened with flash-kernel-installer, a hacked in workaround that makes flash-kernel check for an existing file which only flash-kernel-installer puts in place was temporarily added with the flash-kernel 3.0~rc.4ubuntu14 upload in quantal
