Libre office calc icon not showing in Launcher properly

Bug Description
1) Log in to Ubuntu with Unity 3D.
2) Open a LibreOffice Calc file(.ods).
3) The LO Calc icon is sometimes not displayed on the Launcher, instead an empty space is displayed(refer to the attached screenshot). Sometimes, the icon is present, but there is an empty space on the launcher just above the Calc icon.
Additional Information: I logged in to Unity 2D for a brief period to check if the issue occurs there also, but it worked fine there.
