Sound through headphones randomly stops working

Bug Description
Sometimes, apparently randomly, sound stops working through the headphones. Sound still works normally through speakers but not through headphones. Pulseaudio correctly senses the headphone plug/unplug and the device appears in the output devices list but no sound is heard.
I've tried killing pulsaudio and restarting the session but only a reboot solves the problem.
