xen losetup failed

Bug Description

Hello,

We are running 4.1.1-2ubuntu4.2 on Ubuntu 11.10 amd64.
Our setup:
- 1 san server
- 1 ubuntu xen dom0 with iscsi/multipath configured & connection to our san.
We're working with raw disk images & those are stored on the san.
The virtual disks are used in the xen.cfg with "file:"
- max_loop=256 & not all loop devices are in use.

**Problem:
When we do a xm create vm.cfg, sometimes the process stops with following message:
error: Device 832 (vbd) could not be connected. losetup /dev/loopX /xen/domains/X/X/disk.img failed
- When we check with losetup -a, the /dev/loopX isn't in use.
- When we, after the error, try to create the vm again, it starts with no problems.
