TOC element displays empty line if a doc or folder doesn't have a title

Bug Description
If no title exists it should fall back to the id, so there is something to click.
More fun would be to insert a hysterical warning: "No title! Author Name did
it!! Inform the webmaster!!!"

This is fixed as of Silva 2.0, due to my AutoTOC refactoring into the tocrendering adapter.
