Bug Description

Expected: Headphones recieve audio & system speakers are muted when headphones are plugged in

What Happened: Headphones recieve no audio & system speakers are muted when headphones are plugged in

Ubuntu Release: Ubuntu 12.04LTS x86_64
Hardware: Asus G73sw with Realtek chip using Intel HDA codec

NOTE: A fix does exist. You need to edit the following file:
/usr/share/pulseaudio/alsa-mixer/paths/analog-output-headphones.conf

and change the following line:

[Element Speaker]
switch = off
volume = off

to:

[Element Speaker]
switch = mute
volume = zero

This is because my hardware, using the alsa intel HDA driver, uses the speaker channel as part of the headphone path (A similar fix is in the configuration file already for systems that use "front" as part of the headphone path).
