Cannot convert to epub

Bug Description

I have tried converting my filtered HTML file to epub. All I get is the cover image and a blank page. I've used the same HTML file to create a mobi and lit version, and those both work.

I'm using 0.8.51, using Windows XP

Your HTML file contains all its text inside <form> tags, <form> tags are not
allowed in epub, and so calibre removes them. Remove the form tags and you will
be fine.

 status invalid

Thanks -- I have NO idea how those form tags got there. I don't know what they are, and wouldn't know how to add them if I tried. But taking them out solved the problem.
